A comprehensive 1-on-1 consultation focused on understanding your medical history, current concerns, and long-term health goals. This visit allows us to establish care, review any ongoing needs, and develop a structured plan moving forward.
May include:
- Full medical history review
- Preventive screening and bloodwork planning
- Discussion of current concerns and future care needs
This visit serves as both an initial meet-and-greet and a comprehensive medical assessment.
Must provide proof of past prescription.
No controlled meds including opioids, benzodiazepines, stimulants, or Specialty Care.
$25 fee for each additional medication refill.

*** Can be upgraded to rostering intake visit for additional fees ***
One medical concern per visit:
- Minor illness (cold/flu, sinus infection, pink eye, UTI, rashes, bug bites, allergies etc.)
- Musculoskeletal injuries (e.g. sprains)
- Medication requests, management, or extended prescription refills (non-specialty meds)
- Lab or imaging request
- Specialist referral
- Sick notes for school or work*
- Medical forms or documentation* (may require additional time and fees)
- Insurable services/devices (physio, massage, chiro, orthotics, etc)*
This visit may include:
- Travel vaccine review and recommendations
- Prescriptions sent directly to your preferred pharmacy in Ontario
- Personalized travel medication planning
Common travel prescriptions may include:
- Prevention and treatment for traveler’s diarrhea
- Immunization injections
- Antibiotics and other destination-specific medications
- Support for altitude, motion sickness, or other travel-related risks
Care is individualized based on your destination and medical history to ensure safe and well-prepared travel.

This package includes TWO in-depth virtual visits focused on:
- Detailed assessment and diagnosis of ADHD and related concerns
- Review of screening tools completed prior to visits
- Discussion of treatment options, including medication
- Initiation of appropriate therapy
Additional details:
- Prescriptions can be sent to any pharmacy in Ontario
- Lab work may be arranged if indicated
- Medical, school, or workplace documentation can be completed during visits*
Ongoing care is provided through Virtual – Specialty follow-ups, typically every 1–3 months.

- Men’s health (erectile dysfunction, premature ejaculation, testosterone, libido)
- Weight management (including GLP-1 medications)
- Hormone therapy (menopause, testosterone)

- Longevity lifestyle medicine

Form completion fees: $50 per page
Medical sick notes: $25 per note
Insurable services/devices prescriptions: $25 per prescription
Home-visits are currently at full capacity and not accepting new home-visit patients at this time. Updates will be provided when availability resumes.
